I agree and provide this written instruction to NSM Music Inc or its designee (and any assignee or potential assignee thereof) authorizing review of my personal credit profile from a national credit bureau. Such authorization shall extend to obtaining a credit profile in considering this application and subsequently for the purpose of update, renewal or extension of such credit and for reviewing or collecting the resulting account. This authorization shall be as valid as the original.

By signing above via the (E-Sign Act) the individual(s) as principal of and/or guarantor for the applicant, represent that all information provided with this Application is true and correct and hereby authorize NSM Music Inc to obtain from third parties, information it deems necessary to arrive at a decision regarding this Application. To help fight terrorism and money laundering, the information you provide will be verified.
